The templates for the H&P and Progress Note can be accessed with dot-phrases “.FPCINTAKE” and “.FPCPROGNOTE,” respectively. A note must be completed for every patient visit within 24 hours. These notes will be audited by the FPC Director. The patient note should state that the internal supervisor agrees with the treatment plan and at least one note should be sent to them for co-signature monthly.
